Bilingual self-guided course for a first-year Cornell student observer

Self-Guided ENT OR Observation

ENT Anesthesia: Shared Airway and Pediatric Considerations

This course is designed for independent reading: use tomorrow's 9-year-old and 7-year-old pediatric ENT cases to understand the core idea of ENT anesthesia: the anesthesiologist and surgeon work around the same airway.

ENT anesthesia roadmap
Start with the roadmap: pre-op assessment, airway plan, induction, shared-airway surgery, emergence, and PACU monitoring.

Start Here

First Principle: The Airway Is Both Lifeline and Surgical Field

In many operations, anesthesia manages the airway while the surgeon works elsewhere. ENT surgery is different: the surgical field may be in the nasopharynx, oropharynx, larynx, or glottis. The anesthesia team must maintain oxygenation and ventilation, while the surgeon needs access and visibility. Communication is not optional.

Shared airway triangle
This is the central concept figure: patient airway, ENT surgeon, and anesthesia team form a working triangle.

ENT Anesthesia Features

What Makes ENT Anesthesia Different?

Airway management and surgery happen in the same space.
Blood or secretions can affect the field and airway.
Laryngeal stimulation may trigger coughing, breath-holding, laryngospasm, or hypoxemia.
Shared airway hand-drawn table metaphor
The hand-drawn figure explains the shared airway as a small table: both teams need it, space is limited, and communication sits in the middle.

Tomorrow's Cases

Tomorrow's Procedures: Build the Anatomy Map First

These cases all sit near the upper airway, but the exact location changes the anesthesia focus. Ask: is the lesion in the nasopharynx, oropharynx, laryngeal inlet, or glottis? Could it affect ventilation, intubation, bleeding, or emergence?

Focus: nasopharyngeal obstruction, mouth breathing, snoring, and OSA risk.
Focus: oropharyngeal space, bleeding, aspiration concern, and post-op pain.
Focus: airway narrowing, tube passage, post-op edema, and backup airway plan.
Focus: glottic-level surgery, small tube, cough avoidance, and vocal fold protection.
Tomorrow's ENT case map
This figure helps connect the four clinical labels with their English anatomy locations.

Pediatric vs Adult

Children Are Not Just Small Adults

Children have smaller airways, so mild swelling or secretions can create a larger increase in resistance. They also desaturate faster because oxygen reserve is limited relative to demand. In 7- and 9-year-old children, the differences are less dramatic than in infants, but ENT surgery still concentrates risk around the airway.

Memory line: pediatric ENT anesthesia is not low attention just because the surgery is small; the issue is small airway + airway surgery + emergence stimulation.
Pediatric versus adult airway infographic
Compare pediatric and adult airways: space, oxygen reserve, equipment size, and drug dosing all need pediatric thinking.
Small airway tunnel hand-drawn metaphor
The tunnel metaphor shows why the same swelling matters more in a smaller airway.

Adenoid Hypertrophy and OSA

Adenoid Hypertrophy: From Snoring to Perioperative Airway Risk

Adenoid hypertrophy may cause nasopharyngeal obstruction, mouth breathing, snoring, and sometimes obstructive sleep apnea (OSA). This affects pre-op assessment, opioid caution, emergence planning, and PACU monitoring.

Adenoid hypertrophy and OSA risk
The goal is not just to remember enlarged adenoids, but to understand how they change oxygenation, ventilation, and recovery monitoring.

Lesions and Vocal Cord Polyp

Tonsil, Larynx, Vocal Cord: Same Airway, Different Risk Profile

A tonsillar mass mainly affects oropharyngeal space and bleeding risk. A laryngeal mass is closer to the critical airway inlet and may affect intubation or ventilation. A vocal cord polyp sits at the glottic level, where exposure and smooth emergence matter to reduce coughing and vocal fold irritation.

ENT lesion anesthesia risk comparison
This comparison is useful before the case: the anesthesia plan changes with lesion location.

General Anesthesia Workflow

PACUPediatric ENT General Anesthesia: From Preoxygenation to PACU

A practical observation sequence is: confirm fasting, screen for URI and OSA risk, preoxygenate, induce anesthesia, establish IV and airway, secure the tube, communicate during surgery, aim for smooth emergence, and monitor in PACU. Specific drugs and airway choices are clinician- and case-dependent.

Pediatric ENT anesthesia workflow
Observe in this order: why does the anesthesia team do each step?

Emergence Risk

Emergence Is Not the End; It Is a High-Attention Transition

During emergence after pediatric ENT surgery, watch for laryngospasm, coughing, hypoxemia, bleeding, secretions, agitation, pain, and nausea. Extubation timing, suctioning, analgesia, antiemesis, and PACU handoff all matter.

Emergence wind gate hand-drawn metaphor
This figure draws emergence as a wind gate: the patient is waking up, but the anesthesia team must stay highly focused.

Student Observation Route

How to Observe Independently: Six Checkpoints

Age, weight, fasting, URI, OSA, snoring, allergies, previous anesthesia history.
How is preoxygenation done? How is the child reassured? Is ventilation stable after induction?
How is tube size chosen? How is tube position confirmed after securing?
When does surgery need more space? When does anesthesia need a pause or adjustment?
Is suctioning done before extubation? How are coughing, laryngospasm, hypoxemia, and agitation managed?
Handoff includes airway events, bleeding, analgesia, antiemesis, SpO2, and monitoring priorities.
Student observation route
This figure is useful before entering the OR: follow these six stations during observation.
